I can see it happening, if your step mom is hot and just a few few years older than you.
This is actually the only way in some cultures, and happens more often than many would imagine,happens more casually with best friends deceased wives/husbands too ..

I was talking to a friend yesterday, he told me a friend of his, (knew he was dying to cancer) wanted him to take care of his wife. They actually have a relationship but they are not living together.

Yep and myself and a friend had similar discussions,lighthearted of course,he told me he prefer me have her if he dies...was joking about it in front of her,awkward!!

What outrage? It's not everyday someone is sexually involved with his sister-in-law (not in the open).
The issue is magnified by the fact that they are well known.
The outrage might be because he left his wife for her.
