Chase problem is technique. He pushes his left foot down the track and then brings his right foot across towards off stump. His waist rotates with his right foot and that leaves him totally off balance to anything outside off.

He has been failing for a long time in all formats

It is a technical flaw, sometimes they are difficult to correct. Nothing is wrong with his eyes.
