The only reason that 7 ball overs seldom occur at the international level nowadays is because de umps are in contact with the third ump who alerts them.
The on field umps are supposed to signal each other with two balls and one ball remaining in the over but sometimes when there are several incidents in an over a loss of concentration can happen.
It is also recommended that a ball is counted after it is dead but some people have trigger fingers and count on delivery and since most if not all counters only move forward, they rely on remembering not to count the next ball if the one before was not a legal delivery, instead of advancing the counter six times.
