One time I remembered Desmond Haynes was late returning to the field after the Lunch break. He came back on the field smiling and grinning while still tucking in his shirt in his trousers. The commentator said he needed an extra bathroom break
So to answer your question:
Fielding Team
Player can leave but you will be 1 player short. For a keeper they can switch out but it take someone to pad and glove up with the Umpire's (and Opposing captain's) approval.
“Under Law 2.2.3, substitute fielders shall only be permitted in cases of injury, illness or other wholly acceptable reasons.
‘Wholly acceptable reasons’ should be limited to extreme circumstances and should not include what is commonly referred to as a ‘comfort break’.”
Batting Team
Umpire discretion:
While there is no formal provision, in rare cases, an umpire might allow a batsman to briefly leave the field for a critical need if they inform the umpire beforehand and it doesn't significantly disrupt the game.
